Instructions:
Welcome to the Department of Mathematics web forms site. The link above will take you to a sign-in page for a database that has been published over the web and through which you will be able to fill in many of the standard forms that you often see in paper form (committee wish list, course request form, teaching preferences form, etc.). We are testing this environment as a way to eliminate paper submissions and also to have information you provide placed immediately in a database for easy retrieval.
When you initially sign into the database you will need a username and password. The initial choice of these follows the following example:
John BubbleheadDoll who has e-mail address bubblehead@math.uiuc.edu or bubblehead@illinois.edu has:
username: bubblehead
initial password: bubblehead
On entering the database you will be taken to a page where you can select a form to fill in and also to some instructions for changing your password.
Always remember to press the SIGN OFF button on a form before quitting your browser.
The server mechanism we are using can accept up to 5 simultaneous connections at any one time, so if you are denied access at one time, try again sometime later. And always remember to press the SIGN OFF button on a form before quitting your browser. This signals the database that you are no longer a current user and so others may sign on.
